CPS - Referring a Patient


We are scheduling new patient appointments at 855.615.PAIN (7246) (Toll Free for New Patient Only) and our referral fax number is 888.295.1610 or you can email us at referrals@cpspain.com.


  • We offer fast track procedures. Simply check the procedure only box on your referral form and we will get them in immediately for procedure only. We handle the pre-cert.
  • We return the patient back to you in addition to sending progress notes.
  • We provide quality care and timely consultations.
  • We send follow-up communication.
  • We offer 45+ locations throughout the Southeast for the convenience of your patients.


  • Phone Calls: If you call in to schedule your patient’s appointment, it is scheduled right then.
  • Fax, P2P, or online referral submission: Once we receive your referral, we will reach out to the patient within 24 hours to schedule their appointment. Once the patient is scheduled, we will send their appointment details via fax back to your office. If for some reason we cannot schedule their appointment or they have declined, we will send a letter back stating that.


  • Demographic Sheet
  • Any applicable imaging
  • Insurance card or Work Comp/MVA information
  • We will request any records or discharge letters if needed

Our physician liaison team is designed to increase physician referrals, strengthen relationships with referring providers and staff, and provide valuable customer service. They serve as a personal contact for referral sources. At the heart of the program lies our well-trained physician liaisons. They are available to speak intelligently and promote clinical services and treatments, as well as address questions and concerns as they arise.
